What can the Care Navigators help me with?

We all need help from time to time, but we don't always know where to find it, or who to ask


Our Care Navigation service can help you with a wide a range of problems..........

You might have money worries keeping you awake at night, you might be struggling to cope with a bereavement, you might be going through a family or marriage breakdown, you might be lonely and feeling isolated or you might be finding it increasingly difficult to care and support a relative or friend................Whatever you are struggling with you probably feel that your consultation with your GP or Practice Nurse does not seem long enough to discuss some of these things, or perhaps it just doesn't feel right to 'bother' the GP with things that are not medical. 

At CGMG we recognise that these difficulties you face have an adverse effect on your health and well-being and getting the right help could have a major impact on your life. We have therefore invested in CARE NAVIGATION resources for you to access.

The first element of this service is this CARE NAVIGATION section of the website - it provides useful links and advice that could help you start to access the help you need to change your life. You will find help and advice covering the following:

  • Stress and mental health issues
  • Bereavement and loss
  • Money worries
  • Loneliness and isolation
  • Support for Carers

The second element of this service is the trained CARE NAVIGATORS that we now have working in the Practice. Lisa, Lynsey and Julie have been trained to work together with patients to help them identify what would make a positive difference to their life and signpost them to the services best suited to help.
