How to join the practice

Joining the Practice is quite simple - there are some forms that we ask you to complete and we also need to confirm your identity

The majority of our patients live within our practice boundary which includes the following areas:

All postcodes within NE8 and parts of NE9, NE10 and NE11 - our reception team can confirm whether or not your address is within our boundary.

Patient Registration 15 and under

 

All newly registered patients also need to complete a GMS1 form - and these are only available from reception.

Don't worry if you live outside the Practice boundary but would still like to join us - from January 1st 2015 this is now possible*. There are some circumstances when we might decide it is not appropriate accept a  patient onto our list if we feel we cannot offer the level of care you might need. If this is the case, we will discuss with you.

When you register we will ask you to bring with you proof of your address - this could be a recent utility bill, or bank statement etc. If you require regular medication you will need to make an appointment with a GP for your initial prescription.

If you would like help or advice on sopping smoking or your alcohol intake, please make an appointment with one of our Health Care Assistants.

If you wish to request access to our online services, we will also need photo ID, such passport, or driving license.

*We are not able to provide Patients who live outside our boundary with Home Visits. All patients registered with the Practice must be resident in England.
